In order to request Medical Records from Children's Clinic, please use our Medical Records Form.

 

• To have medical records released TO Children's Clinic.
• To have medical records released FROM Children's Clinic.

 

These forms are also available at all of our locations if you are unable to download and print them. Please call for any medical records questions.
